August 18 2015 I was working at the weigh station . I was taking care of a customer, when I sat down in the office chair not noticing it was all the way up. The chair rolled out from under me, and I hit the concrete floor. I injured both my neck and lower back.
The doctor Kevin Colbert set up physical therapy for me, I went 3 times a week.

I am in continuous pain for 5 years, and have gone back to Dr. Culbert several times. A simple movement can put me in excruciating pain--example just turning over in bed, or driving for long periods of time. The last visit July 2017, Dr Colbert was to set up another round of physical therapy-that didn't happen -Denied. July 8 2019 I went back to Dr Colbert, I sat there for hours waiting to get approval from York Risk Services. After hours sitting there, I walked up to the receptionist asked her if I could see a doctor if I paid for it myself. I was called in to see the doctor (not under workman's comp) while sitting there Carmin walked in with the paper work that workman's comp would take care of it. Prior to that, the physician said he would give me something for the pain. He sent me to get x-rays. After the x-rays, Dr. Anil Suwal told me he could not give me any pain meds under workman's comp to go buy some Advil. Follow-up in 5 days. That was denied.

I came home called York Risk Services talked to Samantha Dejesus. The first thing out of her mouth was "we are not going to cover it since it is a reinjury." First of all I did not reinjure my back or neck, and I don't appreciate being called a liar. I have not seen a doctor for this other than Dr. Culbert. Samantha Dejuses stated "if I need a doctor to go to the emergency room, and pay for it myself."
I called Workman's comp she affirmed to me that I was awarded life time medical care for my neck and back injuries.

Samantha Dejuses continues to believe I am lying about reinjuring to her. She has informed they are taking me to court.
