      Fair Haven Community Health Center, established in 1971 and located in New Haven, Conn. is a nonprofit organization. The center specializes in chronic disease management and health disparities. Its clinical staff includes certified physicians, pediatric physician assistants and family practitioners. The center offers health counseling programs, laboratory services and recreational activities. It also provides pediatric services. The center also provides HIV care and testing services. It is affiliated by the Yale-New Haven Hospital and approved by Joint Commission on Accreditation of Healthcare Organizations. The Center participates in various commercial insurance plans, including Anthem Blue Cross, Oxford, Cigna and Health Net. In addition, the center also participates in the Community Health Network.
