- General SurgeryReducing Patient Choice Among Qualified Cosmetic Surgeons. There are many extremely qualified cosmetic surgeons who are not board-certified in plastic surgery. In fact, many cosmetic procedures were developed by physicians from specialties other than plastic surgery. Residency programs, whether in dermatology, general surgery, oral and maxillofacial surgery, otolaryngology or plastic surgery, do not encompass significant training in the vast array of cosmetic procedures and certainly do not provide adequate training to render a graduate prepared to practice cosmetic surgery. Surgeons develop their cosmetic surgical skills through post residency training and experience. Accordingly, to identify the most qualified surgeon for any given cosmetic procedure, one must look to and compare surgeons' post residency training, experience and practice history. Therefore, a patient may find that the most qualified surgeon available in their community to perform a breast augmentation is a board-certified general surgeon; the most qualified surgeon available to perform a chemical peel may be a board-certified otolaryngologist; or the most qualified surgeon available to perform cervicofacial liposuction may be a board-certified plastic surgeon. The point is that a cosmetic surgeon's skill and ability will depend on the surgeon's cosmetic surgery training and experience, not on his or her core board-certification. Consumers led to consider only board-certified plastic surgeons rely on irrelevant criteria and, accordingly, have their choice among qualified cosmetic surgeons greatly reduced. *
- Hand SurgeryMost physicians have a few procedures in which they are the most experienced. A good place to start is with a doctor who specializes fulltime in cosmetic surgery. Some only perform a few types of surgery. These procedures (noses and liposuction in particular) have learning curves of up to 1,000 cases. The standing joke among surgeons is that the doctor should pay the patient for the first 100 nose surgeries he performs, then the patients can start paying the doctor. Experience is almost everything. A surgeon who also provides surgery for automobile accidents, hand surgery, or other procedures unrelated to cosmetic surgery, may not be what you are looking for.

- Brow LiftSome people have a very narrow space between their eyebrows and eyelashes and they can benefit from having a little bit — sometimes only 1/8th of an inch — of elevation of their brows. Brow lifts go along with eyelid lifts because a careful brow lift can make an eyelid lift much more spectacular. The modern brow lift is done through a few incisions in the scalp. Brow lift patients do have some pain after the procedure and may need codeine-type painkillers for about 2 days after the surgery.

- Face LiftCollagen has been used for many years as the standard facial filler. However, its expense and brief life span (three to four months at most) have been very frustrating for patient and doctor alike. Our new facial fillers can last a year to several years, and many of them have no known allergic potential, unlike Collagen. Lips, cheeks, the folds around the mouth, and chin areas are all areas where fillers are used, and it's called the "Liquid Facelift" when we use the proper amount in the proper way for the right patient. The field of fillers is changing more rapidly than the others, thanks to our pharmaceutical industry, and currently we offer JUVÉDERM® Injectable Gel, Radiesse®, fat, and others. Fillers for the forehead creases are slightly more risky because there are veins that communicate with the brain, and strokes have been reported (very, very rarely). But for the most part, the only risks are lumps, allergy (as yet unreported with any of the fillers we use) and inflammatory nodules ("granulomas": very, very rarely). Most lumps made by fillers respond to massage, or in some cases need to be injected or very rarely removed. Consult with your doctor about your individual case.
- BlepharoplastyA lower eyelid blepharoplasty can be done with mesotherapy by injecting fat-shrinking medication into the lower lid's fat. This takes only a few moments, although there is some swelling for several weeks and the patient should generally be taking some cortisone prior to the injection in order to minimize this. The treatment can be repeated at six weeks if the fat shrinkage is not adequate after the first injection. This can often produce a fairly flat lower lid. No treatment of the skin is necessarily done at the same time, however, and wrinkles of the skin can persist if the skin is not peeled in some fashion.
- Abdominoplasty"Tummy tuck" has become a household word, but the word hasn't gotten around about the problems associated with abdominoplasty. When a person comes to a surgeon's office and asks for a tummy tuck, nine times out of ten, that's exactly what she gets. Most patients desire an improvement in their tummy and assume a tummy tuck is the best way to achieve it. But the unsightly 20+ inch incision in the lower tummy is often not fully understood until after surgery, and is frequently concealed in the photographs the patient sees before the procedure (see Internet ads ).

- Laser ResurfacingSome of our Visalia and Los Angeles cosmetic surgery patients need other eyelid procedures to smooth out the wrinkles of the lower lid. There are two options: laser resurfacing (laser peeling), or taking a little pinch of skin from the lower lid near the lashes. Lower eyelid surgery without skin resurfacing has a much easier recovery, but the results aren't as consistent. Some patients choose to have their eyelid surgery and come back later to have resurfacing when they have more time. Eyelid surgery patients should use ice compresses for the first 24-hour period and most require only two to three days off work if they don’t have resurfacing and about a week if they do.
- GynecomastiaUsually starting in puberty, some men begin to see their breasts take on a more female shape and size. A hard, sometimes painful lump of breast tissue grows under the nipple area, and weight gain generally causes the condition to worsen as fat over the chest wall muscle accentuates the size of the breasts. For other men, gynecomastia begins to develop later in life as a result of taking certain drugs such as steroids used to increase muscle mass.
